School Forms & Enrollment

Have you had a tour of the school and are ready to take the next step? Enrollment requires some paperwork from parents. The school needs to collect basic information about the you and your family, some forms are required by the State of Ohio, and, most importantly, others help us get to know your child better. Most of the forms are available here on the website, the forms can be emailed or mailed to you, or you can print off the enrollment forms. In addition, we have Open Houses and we hold a Orientation Day before the start of each school year.
